New Player Registration!!

New Player Registration
 What should you bring???
 Bring a completed registration form found here...

Bring a 3X5 picture of your child.
(School pictures work well.)
Bring a copy of your child's birth certificate.
Bring a current copy of your child's report card.
(This is for proof of grade level. We are not interested in their grades.. no worries!!)
Bring a copy of your child's last physical.
(Before your child can practice, we will need one dated after 8/01/2015)
  Bring a check dated 6/1/2016, made out to PYFC for ...
                                            $175: For football
                                            $150: For cheer
                                              $75: For Flag football 

Your child does not need to come with you this night, as long as they 
have filled out their portion of the Registration form.
 (Where it asks for their signature.)

Hello PYFC Families! 

We are trying something new for 2016 - which will hopefully save us all some time.  
If your child participated in PYFC in 2015, you are eligible to pre-register any child in your family for 2016. 

Our A, B, and C Football teams will be limited to 28 participants.
Our FLAG Football program will be limited to 16 participants.

In order to ensure your child has a spot reserved for them, you must do the following:

#1 - Fill out the form below and click "SUBMIT" - THIS WILL HOLD YOUR CHILD'S SPOT UNTIL 2/15 OR YOUR PAPERWORK IS RECEIVED.  ON 2/15 - ALL SPOTS THAT WE DO NOT HAVE PAPERWORK FOR WILL BE OPENED UP TO FAMILIES NEW TO THE PROGRAM.

#2 - Collect your paperwork:    

     Bring a 3X5 photo of your child
(A school picture works well.)
    Bring a copy of your child's last physical.
(Must be dated after 8/1/2015, to particilate in the first practice)

Bring a current report card 
(For proof of current grade level, we are not looking at grades..no worries!)

                  (Returning player should have one on file)

 You will only need to fill in: 
Participant Name, Player/Cheerleader Signature, Parent Signature on Page 2...
       We will print out the rest of the information you entered in the form above,
 & attach it for you!

 Payments are NON-REFUNDABLE after that time.
If you change your mind about participating...
 you must email penfieldyouthfootball@gmail.com,  prior to 6/1/2016.  
Exception:  A Squad Cheerleaders that make JV Cheer.

      Drop it off at our returning player registration on Saturday, February 6th from 8-10 AM at the Penfield Community Center.

REMEMBER - YOUR CHILD'S SPOT IS NOT SECURED UNTIL YOU HAVE COMPLETED ALL OF THESE STEPS.  

PAPERWORK MUST BE IN HAND BY 2/15 IN ORDER TO SECURE YOUR CHILD'S SPOT.
